Patient  centered medical homes can offer ”High touch- High-tech” person centered care for vulnerable populations offering a perspective of leveraging information and optimal IT functionality to deliver improved clinical  outcomes,  patient satisfaction  and decrease provider burnout. The opportunities for global initiatives to improve health and well being and train the healthcare providers of th 21st century are yet to be fully appreciated.
