The Community Resource Network (CRN) is a person-centric, SDOH-focused Community Information Exchange (CIE) which is integrated with the Quality Health Network (QHN) Health Information Exchange (HIE), and gives Medical, Social Service, and Behavioral Health providers a rich, shared, 360˚ ‘whole person’ view to better communicate, collaborate, share and analyze data, and improve client outcomes.
 
The system’s self-organizing Care Team model features lightweight yet powerful Care Coordination and Consent tools that reduce the friction and burden of navigating through the healthcare and social service ecosystems – essentially ‘filling in the gaps’ of a process that is vastly silo’d and fragmented today.
 
Dick Thompson, QHN CEO, will discuss at a high-level the CIE Platform’s value proposition for its stakeholders based upon four key capabilities:
 
  • Its ability to collect and attribute data (e.g. diagnostics, encounters, care plans, assessments, etc.) via a wide variety of modalities on clients in need of services, as well as attribute those services to the purveyors of such services from any or all of three broad domains: social, behavioral, and physical health
  • Stratify/identify the population of individuals most at risk across the three domains through predictive modeling and flexible reporting capabilities
  • Facilitate the community-wide (or region-wide) referral and coordination of services and/or interventions across all three domains for those at risk or in need of such services
  • Report on the outcomes of those services in such ways that sustainable funding for ongoing Community Information Exchange operations can be achieved
