Barbara McAneny, MD, an oncologist/hematologist, and the CEO of the New Mexico Cancer Center will discuss how she developed the “Community Oncology Medical Home” project – an NCQA accredited medical home built around the patient and their support system – that reduced time spent in hospitals for patients and resulted in better care.
Her presentation will focus on the development of a medical home model created outside the hospital, the healthcare IT challenges solved, data analytics tools deployed, and the clinical decision support modeling required to make the program successful.
