Struggling with low Personal Health Record adoption rates, Maricopa Integrated Health System put together a robust plan to increase their activation rate, and drive adoption of PHR features amongst their patient population.  The success of the program put them among the leaders nationally in PHR adoption, and the system has taken that success and pushed use of advanced features to promote patient-centered care and reduce disparities for their safety net population.  This session will describe their approach, their successes, and present insight into PHR usage, while posing opportunities for the future.
