Description
Building on the success of the 2019 social determinants of health data panel, CHI has partnered with Healthcare Innovation again, to deliver an interactive panel and discussion for this year’s program. This session will focus on:
- how community health data is being used to improve care
- how healthcare providers, public health organizations, and community leaders are sharing data to improve care
- a comparison of communities in Colorado with extensive SDOH sharing programs in place verses those that do not have programs (or are in the process of developing programs); and how that impacted their response to the Novel19 Coronavirus outbreak
