Data governance is one of healthcare’s top strategic initiatives. Reliable, trustworthy data is an essential ingredient driving digital transformation, analytic self-service and artificial intelligence innovations, among many other initiatives. However, data governance is not a new concept and many healthcare organizations have tried and abandoned prior data governance efforts – sometimes more than once. The good news is data governance has evolved in some dramatic ways that make new programs far more successful. Knowing how to deploy the right programs, processes, and supporting solutions is the key to delivering measurable value from your data governance efforts. Adding to this complexity, electronic health record applications such as Epic often include some basic data governance capabilities embedded within the application. To avoid “silos of data governance” that can potentially be in conflict or uncoordinated on your path to holistic enterprise data governance, you need a plan and architecture to ensure you avoid duplication of capabilities and any other roadblocks to adoption and scalability. Jefferson University has partnered with Healthcare Innovation to teach our happy hour participants new tools they can use to solve challenges within their organization. This interactive happy hour session will teach attendees how to use design thinking methods to better understand and solve the problems and challenges they encounter daily. You will learn how to identify pain points and create solutions that address important considerations like context and user needs. The session will be interactive and give attendees opportunities to practice these skills as they learn them, as well as ask questions. You will leave this session with a new skill set, outlook, and motivation to create.

The Penn Center for Community Health Workers is a national center of excellence focused on improving population health through effective community health worker programs. Join this session to learn about the IMPaCT model – a standardized, scalable program that leverages community health workers, trusted laypeople from local communities, to improve health outcomes in a wide range of settings. In the last three years, leveraging specially designed tools, training and technology, IMPaCT has become the most widely disseminated community health worker program in the United States; it is being replicated by organizations across 18 different states including Veterans Health Administration, state Medicaid programs; integrated healthcare organizations and even national retailers. IMPaCT has been tested in three randomized controlled trials and improves chronic disease control, mental health and quality of care while reducing total hospital days by 65%. IMPaCT has a $2:1 annual return on investment to payers and has been delivered to over 10,000 high-risk patients in the Philadelphia region. Their website is chw.upenn.edu.
