How Value-Based Care Powers Primary Care and Transforms the Future of Healthcare


Total cost of care measures quality, cost and customer experience and serves as a critical metric in value-based care. During the last three years, SWHR network providers maintained an average annual increase to one-quarter of the regional average. This led to our network hospitals having a 7% higher quality scores than other hospitals and health systems in the region (CMS and USNR). While ranking high in quality scores, our accountable care organization also provided care to the most complex patient base in the state at competitive rates, according to several health plans. In conclusion, by connecting care from preventive and primary to post-acute and home care, SWHR has innovated care delivery to evidence-based practices that improve outcomes at a lower cost.
