In this session, Joe Diver, Director of Digital Health Transformation, Baystate Health, the integrated health system based in Springfield, Massachusetts, will discuss the important work that he and his colleagues have been pursuing to advance the concepts of the digital formulary and digital therapeutics. Diver and his colleagues are leveraging digital health apps to support the care management of patients with such chronic illnesses as hypertension and diabetes, as well as to support enhanced post-surgical recovery for cardiovascular patients. Importantly, the Baystate Health program has been designed to streamline workflow around the prescribing of digital health apps, for physicians.
