Sarah Cho, Vice President of Population Health Management at Integrated Health Partners, a Los Angeles-based clinically integrated network of federally qualified health centers (FQHCs), will share with attendees about the advances that she and her colleagues are making in evolving forward their data analytics to support population health management. Cho will explain how she and her team, at the same time that they build a strong architecture for advanced analytics to support population health, are also continually educating clinicians and staffers in the network’s FQHCs, in order to encourage acceptance of and engagement in, that analytics work, which is so vital to optimizing population health for the vulnerable populations whom they serve.
