Mital Brahmbhatt is a healthcare leader with over 20 years of experience in population health and care coordination. She is known for developing innovative, value-driven programs that improve the patient and provider experience.
Currently serving as System Director of Care Coordination & Population Health for Memorial Hermann’s Population Health Service Organization, Mital leads initiatives supporting approximately 500,000 ACO-attributed lives. Her work focuses on advancing value-based care, analytics, and integrated care delivery across ambulatory settings.
Throughout her career, she has practiced as a pediatric oncology social worker, led ICU operations, built a psychosocial division for a major pediatric cancer center, and developed the STAR Kids Medicaid program—bringing a cross-continuum perspective from payer to hospital to community care.
Mital holds a Master of Social Work from the University of Houston and a Master of Healthcare Administration from Texas Woman’s University, where she also serves as an adjunct professor. She is an AHA NextGen Leaders Fellow and an active volunteer in the Houston community.
