Dr. Matthew Van Patton, Director of Medicaid & Long Term Care for NEDHHS and Dr. Jaime Bland, CEO of the Nebraska Health Information Initiative will walk through the road map of utilizing the health information exchange and PDMP as the backbone of population health strategy for Medicaid and how the evolution of this strategy is assisting in addressing health disparities across the state as well as placing care management tools in the hands of clinicians to manage all populations.
