Truman Medical Centers has empowered a leader within the Pharmacy team to expand the institutions pain management and opioid stewardship services. The pharmacist uses the EHR to review inpatient orders triggered by targeted drug reports, focusing on scheduled and long-acting opioid medications. The Colorado Regional Health Information Organization, one of Colorado’s health information exchanges, is partnering with Colorado Care Partners, a HealthONE accountable care organization, to provide meaningful data for their front-line care management initiatives.
CORHIO is sending real-time notifications of hospital visits (admit, discharges, or ER) for active patients in the ACO’s patient roster. The Colorado Care Partners care managers use CORHIO to gather pertinent data on the patients managed by the ACO and to close gaps in care, recognize rising risk and better manage high risk. In addition, data is used for practice scorecards, which track avoidable ER utilization, transitions of care follow-ups and disease management.

Mountains of useful information, inaccessible to “users.” That problem exists today within every hospital in the form of doctors’ Electronic Health Records (EHR) notes that are not available to patients or available but uninterpretable by patients and their families.
Increasingly patients now have access to their medical information via patient portals however the notes and information are not in a form patients and families can easily understand given they are written with the purpose of communicating with other providers and for documentation. By harnessing the power of the EHR and AI, these records can be translated to language that patients and families can understand with the goal of increased usability of the medical record with the hopes of increasing comprehension, improving provider and patient relationships, and empowering our patients with knowledge about their medical issues and plan. This session will focus on an artificial intelligence pilot conducted by members of the Division of Hospital Medicine at the University of Colorado Hospital that utilized this new technology to simplify patient notes while preserving key content. We will describe this novel technology and patients’ perceptions of the translated note.

| 11:15 AM | The problem of out of date legacy hardware, operating systems and applications across the healthcare industry is endemic. This is especially so at small hospitals and clinics where tiny IT and security staffs and highly constrained budgets, prevent the upgrading of end-of-life and often vulnerable technologies. Aggressive sun-setting of Windows versions by Microsoft and near constant patching requirements compound the pressure on small IT staffs to support and secure their health IT infrastructure. Poor coordination between HIT vendors and Microsoft causes healthcare applications to break if patched or remain vulnerable if unpatched. This situation introduces risk into the healthcare delivery environment as IT systems continue to operate with unpatched CVEs and unsupported hardware and software.
With limited budgets and no panaceas on the horizon, how can CIOs and CISOs of small or critical access facilities get away from continuing to support dangerous legacy hardware and software?
